There is a Medical History document that also requires completion, however, you will need to contact the medical secretaries to obtain this document which can either be posted out to you or sent as an attachment via email.

We have produced a New Patient Information pack which is designed for all new egg recipient patients and explains what paperwork we require, the various tests a recipient needs to have, the initial services we provide and maps/directions on how to get to us. If you need a reminder of what test results you need to present upon your initial consultation, we do advise keeping a printed copy of this document to hand.
