Registration for new Egg Share patients to the clinic:
Before attending for your appointment, we will require some forms to be completed and returned to the Medical Secretaries as soon as possible:

There are 2 additional documents that also requires completion i.e. Egg Share registration form and Medical History document, however, you will need to contact the medical secretaries to obtain these documents which can either be posted out to you or sent as an attachment via email.
Once you have completed all these documents, you may either return all of them in an envelope addressed to:

We have produced a New Patient Information pack which is designed for all new egg sharing recipient patients and explains what paperwork we require, the various tests a patient needs to have, the initial services we provide and maps/directions on how to get to us. If you need a reminder of what test results you need to present upon your initial consultation, we do advise keeping a printed copy of this document to hand.
